Disability Case Management

 

A Case Manager coordinates a case plan with the client and their family to identify needs and establish goals. They will provide links to existing services, arrange the purchase of some brokerage services and assist people to develop and maintain links in the community.  Clients will need to reside in the Liverpool, Campbelltown, Camden and Wollondilly areas and be aged 12 to 65. The program is offered for a period of six to twelve months.

 

Priority Access is Given to People Who:

The Junction Works Disability Case Management► Have an intellectual disability

► Have no other agencies involved

► Are at risk of entering the Juvenile Justice system

► Have ageing carers

► Are at risk of becoming homeless
