Young People Leaving Care


The Young People Leaving Care program supports young people with an intellectual disability, aged 15 years and over, who are in the care of Children’s Court for 12 months or more and are requiring continued support through ADHC services.

 

The program supports young people who reside in Liverpool, Campbelltown, Camden, Wollondilly, Wingecarribee, Fairfield and Bankstown LGA’s.

 

The Program Provides Case Management and Daily In-Home Support To Assist You to Address Issues Such As:

► Where you will live and who with?

► What sort of work or study you will do?

► Where your money comes from and how you to manage your budget and pay your bills?

► What supports you may need to help you live independently?

► What activities and recreations and social networks you would like to maintain and how?

► What assistance you may require to manage any health and ongoing medical needs?

 

 

Referrals are only received through Community Services and ADHC.
