The Youth Case Management program provides complex case coordination for young people and their families. It provides group workshops for community organisations (such as schools) to address identified youth specific issues which include, bullying, confidence building and skill development.
The Youth Case Management Service supports vulnerable young people aged 12 to 17 years, and their families residing in the Liverpool LGA.
Priority is Given To Young People at Most Risk Of:
► homelessness or living in youth refuges
► care of Community Service
► contact with the Juvenile Justice system
► violence or other abuse issues
► drug and alcohol abuse
► experiencing mental health and/or other special needs
► having difficulties with school participation.
The Youth Case Manager will work collaboratively with you, your family and carers to identify goals and strategies to address your needs in the short and long term. Anyone can make a referral on their own or on another’s behalf.
